To request a copy of your medical records, you must complete and sign an Authorization for Release of Medical Information, which is downloadable on this page. You can drop off a signed copy at the BAPP office or fax it to 510-597-7154. General turnaround time for medical records is 48 hours after receiving your request.

For requests greater than 20 pages total, medical records are offered at a fee of $0.25 per page. Once we receive your request, you will be contacted by our staff to coordinate your payment.
