If you are a primary care provider looking to make a referral to our practice, please complete the New Patient Referral Questionnaire (PDF, 36Kb) and gather the information listed in the New Patient Referral Checklist (PDF, 57Kb). These items can be downloaded here. You can fax all new patient information to the BAPP office at 510-597-7154.

If you are a parent who wants your child to be seen by our practice, please get a referral from your primary care provider (pediatrician).

For all MEDICALLY URGENT appointments, please have your primary care provider (pediatrician) call our office at 510-428-3305 and request to speak to the Pulmonologist on your behalf.
